Why do I need to complete a Form R (Part B)

Completion of a Form R annually is mandatory. The Form R (Part B) is a self-declaration form that is part of the evidence required for your ARCP and revalidation. It is required each time you have an ARCP held for you, even if you are currently Out of Programme (OOP). 


Your Form R must include accurate demographic details with the whole scope of practice.   


You will be expected to submit the Form R part B through the TSS system, Trainee Information System (TIS) where all your training data is stored. Paper versions are not acceptable.
